Who are the Service Delivery Team?

The Service Delivery Team offers innovative clinical care programs which respond to individual patient needs not readily met within usual general practice structures and staffing.  This team focuses on services that are being delivered through practice staff or to the patients directly.  Each initiative is designed to add to the overall capacity of the general practice system (not replace or compete with existing services); its programs stem from identified patient needs and improves accessibility to those who might otherwise fall through the gaps.  it includes a range of primary health care programs:

Who are the General Practice Team?

The Practice Centre of Excellence (CoE) is what is traditionally considered the core of Division activity.  It is the heart of what makes General Practice as effective as possible in improving the health of their communities, managing their practices and continuing their ongoing professional development.  The work of the Practice Team centres on:  Systems development, Data Management, Change Management and Business Modelling.  Programs that support general practice, GPs or practice staff to provide an effective service to their patients are included in this team.  These are:

The Communities Centre of Excellence (CoE) is a substantial reconceptualisation of the traditional work of Divisions, by offering innovative social health programs which respond to community needs not readily met within usual general practice structures and staffing.  Programs stem from identified patient needs and improves accessibility to those who might otherwise fall through the gaps.  The work undertaken here presumes a view of heath which is holistic rather than isolated episodes of unwellness.  It includes:
