Lifestyle Modification Program
Overview
The nationally accredited Lifestyle Modification Program (LMP), ‘Reset Your Life, a healthy lifestyle program to prevent diabetes’, aims to reduce the risk or delay the onset of Type 2 Diabetes in participants. The LMP is an integral component of the broader National Prevention of Type 2 Diabetes Program initiated earlier in 2008 as part of the ‘Council of Australian Governments – Reducing the Risk of Type 2 Diabetes’ initiative. The ‘Reset Your Life’ program consists of a series of group motivational and educational sessions supporting lifestyle and behavioural changes and adoption of healthy lifestyle choices in participants.
Programs run over six months and address:
- The risks of diabetes and their relationship to lifestyle risk factors
- The importance of regular diabetes screening
- Nutrition advice and education
- Physical activity advice and education
- Behavioural strategies to support the adoption and maintenance of healthy lifestyle changes
- Smoking cessation and alcohol reduction advice if required and
- Information about community resources to provide support in maintaining lifestyle change.
Objectives
The Brisbane South Division of General Practice’s main objectives in delivering the accredited Lifestyle Modification Programs are:- To orientate practices to the program, client eligibility and screening and Medicare Item referral options
- To support general practice to engage ‘at risk’ clients, establish level of risk and instigate early interventions
- To support practices to interpret LMP referral pathways and follow referral processes
- To coordinate and implement a quality LMP service that includes delivery of the accredited program as per national standards for clients, program data collection, evaluation and reporting and feedback to the referring general practitioner.
Patient Eligibility
To be eligible to attend a subsidised LMP program, patients must meet the following criteria:
- Be aged 40 to 49 years or 15 to 54 years for Aboriginal & Torres Strait Islanders
- Be identified as being at ‘high risk’ for developing Type 2 Diabetes based on the Australian Diabetes Risk Assessment Tool (AUSDRISK Tool)
- Have a diagnosis of Type 2 Diabetes excluded (complete an OGTT)
- Have not attended a subsidised LMP program previously
- Have a GP referral and have provided consent to attend an LMP
AUSDRISK Tool can be self-administered by clients or be completed with the assistance of practice staff or a health professional, providing opportunity to assess client readiness to make lifestyle changes to address identified risk factors.
N.B. The LMP program is currently under review and as result, new patient referrals can only be accepted up until 1st September 2011. MBS Item Numbers
Patients can be referred to a subsidised LMP program through the following MBS Item Numbers:- Item 701: Brief Health Assessment of less than 30 mins; OR
- Item 703: Standard Health Assessment of 30-44 mins; OR
- Item 705: Long Health Assessment of 45-59 mins; OR
- Item 707: Prolonged Health Assessment 60+ mins; OR
- Item 715: Aboriginal and Torres Strait Islander Health Assessment
GPs are required to undertake one of the following health assessments under these items to refer eligible patients to a subsidised accredited LMP:
- 45-year old health assessment
- Type 2 diabetes risk evaluation
- Aboriginal and Torres Strait Islander Adult Health Assessment

Healthy Lifestyle Programs
Lighten Up
Overview
Lighten Up is a group-based healthy lifestyle program developed by Queensland Health. It assists people to:
- Develop a healthy attitude to eating
- Prepare and enjoy healthy food
- Incorporate physical activity in their daily life
- Manage stress
- Learn healthy methods of weight control
- Make long-term behaviour changes to support a healthy lifestyle
Lighten Up is designed for men and women over 18 years of age who want to improve their health, manage their weight and are reasonably mobile. Programs are run by trained health professionals in various locations across Brisbane.
Patient Referral Options

2. Patient Self-Referral - Referral from a medical professional is not essential. Patients can self-refer into a program by contacting the Program Manager (Healthy Lifestyles) on 07 3274 1886 or by contacting the program facilitator directly.
Living Strong
Overview
Living Strong is a group-based healthy lifestyle program for Aboriginal and Torres Strait Islander communities. It is about helping people to make long-term lifestyle changes, like choosing healthier food and drinks and being more physically active.
Living Strong includes a series of workshops which will help participants learn about:
- Healthier food and drink choices
- Healthier cooking
- Ways to become more physically active
- Diabetes awareness
- Improving self-esteem and behaviour change
- Ways to make and maintain healthy lifestyle changes
- Reading food labels
- How to spend their food money more wisely
Living Strong is for most people over the age of 18, but does not cover the needs of pregnant or breastfeeding women. It is recommended that everyone checks with their doctor before joining the program. Participants are welcome to bring their partner, other adult family members or friends.
Programs are usually run by Aboriginal and Torres Strait Islander Health Workers in partnership with other trained health professionals in various locations across Brisbane.
