Lifestyle Modification Program
Overview
The new nationally accredited Lifestyle Modification Program (LMP), ‘Reset Your Life, a healthy lifestyle program to prevent diabetes’, aims to reduce the risk or delay the onset of Type 2 Diabetes in participants. The LMP is an integral component of the broader National Prevention of Type 2 Diabetes Program initiated earlier in 2008 as part of the ‘Council of Australian Governments – Reducing the Risk of Type 2 Diabetes’ initiative.
The ‘Reset Your Life’ program consists of a series of group motivational and educational sessions supporting lifestyle and behavioural changes and adoption of healthy lifestyle choices in participants.
Sessions include:
- The risks of diabetes and their relationship to lifestyle risk factors
- The importance of regular diabetes screening
- Nutrition advice and education
- Physical activity advice and education
- Behavioural strategies to support the adoption and maintenance of healthy lifestyle changes
- Smoking cessation and alcohol reduction advice if required and
- Information about community resources to provide support in maintaining lifestyle change.
Objectives
The Brisbane South Division of General Practice’s main objectives in delivering the accredited Lifestyle Modification Programs are:
- To orientate practices to the program, client eligibility and screening and Medicare Item referral options
- To support general practice to engage ‘at risk’ clients, establish level of risk and instigate early interventions
- To support practices to interpret LMP referral pathways and follow referral processes
- To coordinate and implement a quality LMP service that includes delivery of the accredited program as per national standards for clients, program data collection, evaluation and reporting and feedback to the referring general practitioner.
Program Details
The ‘Council of Australian Governments – Reducing the Risk of Type 2 Diabetes’ initiative aims to address the significant increase in incidence of Type 2 Diabetes in Australia. The Commonwealth Government accordingly introduced MBS Item 713, the Type 2 Diabetes Risk Evaluation or Review, to support general practitioners to address the health needs of people aged 40 -49 years who are at ‘high risk’ of developing Type 2 Diabetes.
This Item provides GPs with the opportunity to review client’s risk factors and initiate early interventions such as referral to the subsidised Lifestyle Modification Programs to reduce the risk or delay the onset of Type 2 Diabetes in eligible clients (as determined by the results of the Australian Diabetes Risk Assessment (AUDRISK) Tool - Score > 15 = high risk).
Clients may also be referred to an LMP through the 45 – 49 year old Health Check (MBS Item 717) or the Aboriginal and Torres Strait Islander Adult 15 – 54 year old Health Check (MBS Item 710). The AUDRISK Tool can be self administered by clients or be completed with the assistance of practice staff or a health professional, providing opportunity to assess client readiness to make lifestyle changes to address identified risk factors.
General Practices may now refer eligible clients to the fully accredited subsidised LMPs being provided by Brisbane South Division of General Practice.
